The membership due is $10 per year.

• ACTIVE membership is for anyone who is DEAF.
• ASSOCIATE membership is for anyone who is HEARING and is a supporter, such as a friend, a family member, or a caring person.

3. Print the form.
4. Fill out the form.
5. Mail the form & payment to the address on the form.
